DECLARATION
of joining the International Forum for Women

I hereby declare I wish to join an association called the International Forum for Women.

I hereby state that I am familiar with the statute and I do accept the resolutions it includes. I shall pay membership fees in the amount specified by the Board and contribute personally to the development of the Association.

The membership fee is 360 PLN annually (paid by the end of January each year); entrance fee - 150 PLN.
